Pricing is very simple and straight forward:

Front Office (membership) forms are $250 per form setup* and $250 per form annually.

Lending forms are $250 per form setup* and $250 per form annually.

*Setup charges are waived if the Credit Union uses Forms Design, Inc. to integrate these documents.

Spanish forms are available for $450 per form setup and $450 per form annually.

We realize that this sounds too easy, but we don't see the need to complicate things by having a different pricing matrix based on either credit union asset size or based on the number of times you print the documents.

The setup fee is a one time fee for each form and includes the rest of the year's licensing fee. You'll be annualized on the next January 1st, which then the annual fees will be due for the entire year, and annually thereafter.

Unlike other companies, there are no additional fees for legal review, no additional fees for each time you print the form, no additional fees based on your asset size. The only additional fee that may be charged is if you want to customize these documents.
